Free Fall on the Runway!

How in the world did the model in the black dress fall through the hole in the runway? I don't understand how no one saw that enormous black hole in the runway after the performance with the dancers left the stage...I think that someone saw it but didn't say anything...which is messed up! She could have really hurt herself!
